Applications are now open for the South East Public Health Practitioner Registration Scheme 2026.
Please read the Public Health Practitioner Registration Scheme Application 2026 Application Pack (PDF) before starting your application. This guidance document contains important information on eligibility, programme expectations, required documentation, and step-by-step instructions to support you in completing the online application.
Application deadline: 11 June 2026 at 12:00pm (midday)
All required application forms must be received by this deadline.
Before you apply
Before starting your application, please ensure that you:
1. Have read the Application Pack in full
2. Meet the eligibility criteria for the scheme
3. Have discussed your application with your Line Manager
4. Have completed the Self-Assessment Form with your Line Manager to assess your readiness for the programme (this does not need to be submitted at application stage but will be requested if you are shortlisted for interview)
How to apply
The application process is completed online using Microsoft Forms. To submit a complete application, you must complete all of the steps below:
Step 1: Complete the Public Health Practitioner Scheme 2026 Application Form.
Step 2: Complete the Practitioner Scheme Equality, Diversity and Inclusion Form.
Step 3: Ask your line manager to complete the Public Health Practitioner Scheme 2026 Line Manager Authorisation form.
Step 4: If applicable, ask your Director or Deputy Director of Public Health to complete the Public Health Practitioner Scheme 2026 Director Approval form.
Step 5: Practitioners will need to submit a current CV to england.publichealthschools.se@nhs.net by 11 June 2026, 12:00pm.
Additional documents
The Self-Assessment Form should be completed before applying, in discussion with your Line Manager, to help assess your readiness for the programme. This form does not need to be submitted with your initial application. However, if you are invited to interview, you will be asked to provide the completed form. The self-assessment is also intended to be a working document that practitioners can continue to update throughout the programme to support their development.
