The Application Process
If you are considering applying to become an Educational Supervisor, please read the Educational Supervisor Guidance before submitting your application. This will provide you with an overview of the programme, explain how foundation dentists and educational supervisors are appointed, dental foundation training payments, educational supervisor responsibilities, standards for training practices and the role of the educational supervisor.

2. If you are reapplying as an Educational Supervisor, please download and complete Reapplication FORM A
3. Application forms should be submitted along with the Self Assessment Form
*New Educational Supervisors will be required to attend the Developing Dental Educators Course. More details and the application process can be found here.
