Fitness to Practice Declarations
If you are subject to any of the following, you must declare these on your application form:
- A criminal conviction or caution (not including driving license endorsements)
- A GMC investigation, proceedings or restrictions
- Employment disciplinary investigation or judgements.
You will then need to complete a FtP Declaration Form which can be downloaded from the Medical Specialty Training website and submit this to the lead recruiter.
Where Thames Valley is the lead recruiting region Forms should be submitted to email@example.com
For the majority of specialty recruitment, applicants will be automatically provided with their shortlisting and/or interview scores via the Oriel system.
